What is endoscopic sinus surgery?

Endoscopic sinus surgery (ESS) is a procedure to remove blockages and treat other problems in the sinuses using an endoscope — a thin, rigid tube with a camera and a light. This type of surgery does not include cutting the skin because it is performed entirely through the nostrils.

ESS is a relatively quick outpatient procedure performed under general anesthesia. During the procedure, the surgeon inserts a thin camera rod with a light on the end (called an endoscope) into one nostril to visualize the sinus tissues and locate the problem. After locating the source of the problem, the surgeon then uses instruments to remove the causes of the sinus blockage, such as nasal polyps, scar tissue or mucous membrane swelling. In the case of a deviated septum, special instruments can be used to straighten the septum and reduce the sizes of the bony structures inside the sinuses. Image guidance for the surgeon is often used, which enhances both surgical success and safety. Following surgery, patients usually spend a few hours in our recovery room and most of the times, they feel well enough to go home that same day.

What are some common post-surgery symptoms?

  • Bleeding: It is normal to have some light bleeding 3-5 days post-op. There are several sprays available that will typically stop any bleeding.

  • Pain: Mild pain, such as a dull ache or sinus infection, is common during the first several days after surgery. Extra-strength acetaminophen can be taken to minimize discomfort, as long as aspirin and NSAIDS (such as ibuprofen or naproxen) are avoided. In some cases, a stronger pain medication can be prescribed by your doctor.

  • Nasal Congestion: Nasal congestion and discharge is normal and expected during the first few weeks following surgery. Normal breathing should return within 2-3 weeks.

  • Fatigue: Fatigue is expected during the first week after surgery, so most patients should plan to take 1 week off of work to recover. However, every patient heals differently and some patients may return to work sooner or later.

As patients heal from endoscopic sinus surgery, you will need to follow up with your doctor and allow the care team to clean your nose and sinuses of any blood or fluid left behind. Sometimes these cleanings can be a little uncomfortable, so it is a good idea to take some pain medication about 45 minutes before your appointment.
